Individual Case Management

Bronya Nelson is our case management specialist and liaison between youth and delivery of services. Case management include but is not limited to wraparound support, whereby planning and key personnel involved in treatment can foster team-based, collaborative efforts. Case management is delivered using our three core tiers of service (1) education, (2) skills, and (3) wellness. Case management delivery is person-centered with full utilization of youth engagement in the planning process.
