MY SAFETY PLAN

Confidential client record

This is my plan for getting through hard moments. I wrote it in my own words, and I can change it any time.

1. What Keeps Me Going

People, pets, goals, plans, or anything I’m looking forward to.

2. How I Can Tell How I’m Doing

Changes in sleep, eating, mood, or thoughts. Pulling away from people. What I say or post.

3. Things That Can Set Me Off

4. Things I Can Do on My Own to Calm Down

Music, a shower, a walk, breathing, drawing, a game, writing it down.

5. When Someone Is With Me

6. My People

Friends, family, mentors, or anyone I can call, text, or be around.

7. Places Where I Feel Safe

8. My Angels Nest Team and Care Providers

9. Keeping My Space Safe

We agreed to these steps:

10. If I’m Not Safe Right Now

  1. Tell someone near me, or call the Angels Nest on-call staff.
  2. Call or text 988, any time.
  3. If I might hurt myself, call 911 or go to the nearest emergency room.
